در تست غربالگرى سه ماهه اول باردارى دو نوع اندازه ‏گيرى به همراه سن مادر مورد استفاده قرار مى‏گيرد تا ريسك سندرم داون، تريزومى 18 و تريزومى 13 محاسبه شود.

يكى از اين اندازه‏ گيرى‏ ها با سونوگرافى خاصى انجام مى‏شود كه به آن اندازه‏ گيرى NT مى ‏گويند و ديگرى آزمايش‏هايى است كه روى خون مادر انجام مى‏ شود.

اندازه‏ گيرى NT با سونوگرافى خاصى كه از ابتداى هفته 11  تا انتهاى هفته 13 باردارى  قابل انجام است، صورت مى‌گیرد. در اين نوع سونوگرافى مقدار مايع در پشت گردن جنين كه به آن NT مى‏گويند، اندازه‏ گيرى مى ‏شود. تمام جنين ‏ها در اين قسمت داراى مقدارى مايع هستند كه اين ‌مقدار در جنين‏ هاى مبتلا به‌ سندرم داون تمايل به افزايش دارد. سونوگرافى NT همچنين در موارد زير كاربرد دارد:

  • زنده بودن جنين را تأييد مى‏كند.
  • سن باردارى را تأييد مى‏كند.
  • باردارى‏ هاى دوقلو را تشخيص مى‏دهد.

آن دسته از اختلالات هنگام تولد را كه در اين سن باردارى قابل تشخيص هستند، بررسى مى ‏كند.

در آزمايش خون مادر سطح دو ماده كه معمولاً در خون تمامى زنان باردار يافت مى‏ شود مورد اندازه‏ گيرى قرار مى ‏گيرد: PAPP-A و Free Beta HCG. در باردارى‏ هاى توأم با ابتلاء جنين به سندرم داون، مقدار دو ماده مزبور تمايل به خروج از مقادير مورد انتظار را نشان مى‏دهد.

این نتايج به ما مى ‏گويد كه به چه احتمالى باردارى توأم با سندرم داون، تريزومى 18 و يا تريزومى 13 خواهد بود كه در گزارش غربالگرى به آن ريسك ابتلاء گفته مى‏شود.

 

نتايج غربالگرى سه ماهه اول به پزشک مى ‏گويد كه آيا لازم است تست ‏هاى تشخيصى نظير آمنيوسنتز براى خانم باردار انجام شود يا خير. اكثر زنان پس از انجام غربالگرى نتايجى دريافت مى‏ كنند كه بر مبناى آنها نياز به انجام تست ‏هاى تشخيصى منتفى است، اما به زنانى كه ريسك سندرم داون، تريزومى 18 و تريزومى 13 آنان بيش از ميزان نرمال باشد، انجام تست‏ هاى تشخيصى نظير آمنيوسنتز پيشنهاد مى ‏شود. صرف‏ نظر از سن مادر و يا نتايج غربالگرى سه ماهه اول، تصميم به انجام تست ‏هاى تشخيصى، تصميمى كاملاً شخصى است.

تست ‏هاى تشخيصى، نمونه‏ بردارى از پرزهاى جفتى و آمنيوسنتز را شامل مى‏شود . نمونه‏ هاى گرفته شده براى انجام اين تست حاوى سلول ‏هاى جنين است. آزمايشگاه با انجام تست ‏هايى به روى اين سلول‏ها درمى ‏يابد كه آيا جنين بــه اختلالات كروموزومى مـبتلا است يا خير. انجام آزمايش ‏هاى تشخيصى به طور معمول به تمامى زنــان بــاردار پـيشنهاد نمی گردد، زيــرا احتمال سقط جنین را بالا مى ‏برند، با وجود اين اكثر زنـانى كه تست‏هاى تـشخيصــى را انــجام می دهند دچار سقط يا ديگر عوارض ناشى از تست‏ هاى مزبور نمى ‏شوند و در عمل نتايج تست ‏ها به زنان باردار آرامش و اطمينان خاطر زيادى مى ‏بخشد.

  • انجام غربالگرى دقيق‏تر و زودتر، باعث ايجاد آرامش خاطر در بسيارى از زنان مى‏شود.
  • با انجام سونوگرافى NT امكان تشخيص بعضى از اختلالاتِ بارز هنگام تولد وجود دارد.

اختلالات كروموزومى اختلالاتی هستند كه در آنها تعدادی و يا قسمتى از كروموزوم‏ها افزايش و در مواردى كاهش مى‏يابد و يا در تعدادى از كروموزوم‏ها بازآرايى (چينشِ مجدد) قطعات مشاهده مى‏شود. این اختلالات مى‏توانند بر روند تكامل ذهنى يا فيزيكى جنين تأثير منفى بگذارند. سندرم داون كه تريزومى 21 نيز نامیده می‌شود، شايع‏ترين اختلال كروموزومى است. افراد مبتلا به اين سندرم به جاى 46 كروموزوم، 47 كروموزوم دارند و دچار سطوح مختلفى از مشكلات فيزيكى و ذهنى هستند. احتمال بروز سندرم داون در باردارى زنان در هر سنى وجود دارد اما اين احتمال با افزايش سن مادر افزايش مى‏يابد. در بارداري هاي همراه با سندرم داون مقدار PAPP-A کاهش يافته و مقادير Free Beta HCG و NT افزايش خواهند يافت.
